For your convenience, your appointment will be scheduled for you.  A representative from CLIC or your doctor's office will let you know the exact time. Please follow these suggestions:

1. Be on time. Patients are scheduled one after the other and your promptness will ensure that we are able to serve everyone in a timely and efficient manner.

2. Please be sure to bring your driver's license and all relevant insurance documentation.
